A complete, fixed arch of teeth supported by four implants — placed by a board-certified periodontist, with same-day teeth in most cases.
All-on-4 is a full-arch tooth replacement technique that supports an entire upper or lower set of fixed teeth on as few as four dental implants. Two implants are placed toward the front of the jaw where bone is densest, and two are angled toward the back to make the most of the bone you already have. A fixed bridge is then attached to those four posts — so unlike a denture, it never comes out, doesn't rely on adhesive, and doesn't shift when you eat or speak.
Because the rear implants are tilted rather than placed straight down, All-on-4 often works for patients who were previously told they had lost too much jawbone for traditional implants — frequently without a separate bone grafting procedure first.
You will also see this treatment called All-on-X. The “4” is a brand name, not a rule — the number of implants is set by your bone volume and your bite, and commonly lands between four and six per arch. Dr. Hanasab decides the count from your 3D scan while planning your case, not in advance. Whatever the number, the end result is the same: one fixed full-arch zirconia bridge, not a set of separate crowns placed on each implant.

From first scan to final bridge, most cases run several months. Here's what each stage involves.
A clinical exam plus a 3D scan showing bone volume, bone density, and the exact position of your nerves and sinuses. Every implant position is planned digitally before surgery day. You leave this visit with a written treatment plan and estimate.
Any failing teeth in the arch are removed, usually in the same appointment as surgery, so there's no separate healing wait. The four implants go in per the digital plan — two forward, two angled back. Sedation options are available.
A temporary but fixed bridge is attached to the implants the same day or shortly after. This is the "teeth in a day" part — you go home with a full arch rather than an empty mouth.
Over the next several months the implants fuse to the jawbone. Expect swelling and soreness for a few days, a soft-food diet for one to two weeks, and periodic check-ins so we can confirm the implants are stable. See our post-op care instructions.
Once healing is confirmed, the provisional is swapped for a custom permanent bridge in a durable material such as zirconia, shaped and shaded to your face and adjusted for bite before you leave.

Coverage varies widely by plan. Many dental policies contribute toward parts of full-arch treatment such as extractions, the prosthetic bridge, or a portion of the implants, even when they don't cover the whole case. In most cases, yes. A fixed provisional bridge is attached to the implants the same day as surgery, so you leave with a full set of teeth rather than going without. That first bridge is temporary. Your permanent bridge is placed several months later, once the implants have fused with the bone.
Often not. Angling the two rear implants lets them engage the denser bone at the front and middle of the jaw, which is why many long-term denture wearers who were told they lacked bone for traditional implants still qualify for All-on-4. Some cases do need grafting in specific areas, and your 3D scan is what settles the question.
Four implants per arch is the standard, which is where the name comes from. Some cases call for five or six, depending on bone quality, bite forces, and the span being restored \u2014 which is why the treatment is also called All-on-X. The implants themselves are designed to be permanent, and published long-term studies of full-arch implant cases report high implant survival over periods of up to ten years. The bridge attached to them is a wearing part and may need repair or replacement over time. Outcomes depend heavily on bone quality, oral hygiene, smoking, and keeping up with maintenance visits.
